THIS CONTAINS SPOILERS SO DON'T READ IT IF YOU HAVEN'T SEEN THE FILM YET
This isn’t a review and it’s just my rambling thoughts about The Tethered characters in the film. It does contain SPOILERS so ignore this post if you plan to watch the film, which I do recommend that you to go see because it is good.

From what I gathered the doppelgangers, known as “The Tethered” were originally created by the U.S. government as a way to be in control of the American people (or for labor purposes). But they abandon the project when they realize that the Tethered can’t really function on their own or be that useful while the other half still exists. As we’ve seen them do nothing but mimicking the actions of their counterparts aboveground.
Apparently there can only be one of the same two beings in existence to be in full control (as in control of a soul which makes a person truly whole and complete with freewill, consciousness, sentience, etc.). This would also explain how Jason was able to get his Tethered to walk back into the fire. He figured out how to imposed his ‘will’ or control through their shared soul… I think.
Two bodies can not share one soul which is needed to be in full control and be their own person. Killing the other would make it possible, but apparently switching places works just the same? Because the real Adelaide wasn’t just trapped, she was also forced to be a puppet to whatever the Tethered version of her did aboveground (like being a ballerina, having to pair up with Gabe’s Tethered to give birth to the same children, etc). Her mental state suffered from all that.
So while the real Adelaide lost hope and the control of her soul bit by bit from being stuck underground. The Tethered version of her grew stronger with her newfound freedom. Red eventually gain nearly full control of the soul and figured out how to be her own person. To the point that she forgot that she was originally a Tethered.
However, the real Adelaide still remembered what life was like before. Because she didn’t start out as a Tethered she had enough knowledge to teach the Tethereds a couple of things (which is how they learned about and were fixated on doing the Hands Across America) and she lead them to do what she wanted. To escape and get revenge. That was why she wasn’t so quick to kill the family as she wanted them to suffer first. More so with Red, her Tethered.
Also, the reason Adelaide couldn’t speak that well was because Red crushed her throat. It probably didn’t healed correctly. It’s either that or she didn’t get much use of her voice when the actual Tethereds don’t know how to talk.
